Department: Diabetes Education
Location: University Center for Community Health JOB CLASS NUMBER: 4111 ESTABLISHED:04/09 POSITION SUMMARY/RESPONSIBILITIES Diabetes Nurse Case Manager is responsible to identify best practice management strategies for identified patient populations, strives to achieve cost effective utilization management and best practice care for high volume patients and patients at risk for complex high cost disease/illness that challenge quality of life and are costly to the health care system. The Diabetes Nurse Case Manager has an understanding of UHS/TDI Healths structure and services as well as opportunities/supports available through community agencies. Reports outcomes monthly to the director and participates as a member of the TDI education staff. This position is not unit based and involves the following three dimensions of practice: clinical care; outcomes measurement (tracking, analysis and reporting of clinical, financial, satisfaction and functional outcomes) and process redesign (leading initiatives to facilitate practice based and organizational change). Skills / Requirements
EDUCATION/EXPERIENCE Graduation from an accredited school of professional nursing is required, Bachelors and/or Masters degree is preferred. Three years previous experience in medical/surgical/ICU nursing required, four years preferred. Recent experience as a registered nurse in a diabetes education setting preferred. Adult education experience preferred. Demonstrated teaching experience and knowledge of adult education principles, ability to deliver and evaluate adult education programs. LICENSURE Current license from the Board of Nurse Examiners of the State of Texas to practice as a registered nurse is required. Current certification as a Diabetes Educator (CDE) is preferred. Certified Diabetes Educator credential must be achieved within two years from date of hire. Must maintain valid CPR (BLS) certification. This job posting is no longer available on Jobing.com.
